To help us better meet your individual needs, we have a few information sheets for you to fill out. For your convenience, we are posting those sheets on this website. This way you can print them off an fill them out at your leisure. You will bring these sheets with you when you come in for your free consultation. If you do not fill out the sheets ahead of time, you will simply fill them out as part of the consultation meeting.

LAC Forms
This form contains some general information about the student. We use this information to determine the goals for the student and to create an individualized achievement plan.
This form allows us to contact the student's school/teacher. This form can be completed at any point; however, we are posting it here to give you more time to gather the teacher's contact information.
We take the time to meet with you, at no charge, to put together a plan specific to the student’s needs. You know what you need, and we will do our best to come up with a plan just for you. Because we meet with our students one-on-one, in most cases we can also accommodate your scheduling needs, and we try to work within any deadlines you may be facing.
We look forward to helping you come up with the plan that will result in the success you’re looking for. The combination of a private setting, one-on-one instruction with a qualified tutor, and a plan tailored specifically to your needs are a part of the reason success truly happens here.
